Document Submission Information for All Applicants
All documents must be computer-printed or typewritten. Handwritten applications or documents will not be accepted. Applications must be written in a careful, precise and professional style with correct grammar and spelling to ensure clarity.
In addition to specific narratives and budget documents for each grant program, please submit the following (the number of copies varies; please see each program guideline for the correct number):

Application Form
This must be fully completed with current applicant contact information.
Please do not use a home address for an organization’s address. If the organization does not have an official mailing address, please obtain a post office box or utilize another organization or business for mail.

DUNS Number
Organization or agency applicants must include their DUNS Number. Please go to www.dnb.com for more information and to request a DUNS Number.

Compliance Statement
The compliance statement must be signed by the authorizing official of the organization or by the artist requesting funding. The authorizing official is the person authorized by the organization to enter into contractual agreements. The compliance statement assures that the applicant is not barred or suspended from receiving federal funds from any federal or state agency and attests to the accuracy of the information provided in the application. If the applicant is barred from receiving federal funds, the applicant must submit a detailed explanation surrounding the suspension.

W-9 Form
Each applicant must submit and complete a W-9 form.

Board of Directors’ List
A current list of the policy-making board of the organization with the members of the executive committee identified. Include board members’ complete contact information (name, address, telephone number and email address). Depending on the Kansas Arts Commission program, the board of directors’ list may only need to be submitted one time for the entire fiscal year. See specific grant program guidelines for submission information.

Biographies and Letters of Agreement
If required by the grant program, an organization requesting support for guest artists, administrative or artistic personnel or consultants must include, with each application, one- or two-paragraph biographies of those personnel. If applicable, signed letters of agreement stating the scope of the project, specific duties and total amount to be paid during the fiscal year must also be included with each application. See specific program guidelines for requirements.

Most recent IRS Form 990
(nonprofit, tax-exempt organizations only)
If an applicant is submitting multiple grant applications during the same Kansas Arts Commission fiscal year, only one 990 statement is required. Please check grant program guidelines carefully.

Support Materials
If required by the grant program, applicants must submit support materials. These materials may be information about previously successful programs or projects (e.g., programs, catalogues, newspaper articles, study guides, contracts, artist biographies, etc.). These materials should be carefully selected and no larger than 8 ½ ” x 11”. An identical set of support materials should accompany each application copy for every program or category.
