All of the following documents have been created in Word and PDF formats for your use when completing the Partnership Agreement Grant application. Click on the format that you prefer to download (i.e., Word documents can be used to fill in information directly on the form if you have Word installed on your computer and PDF documents can be used with a typewriter). Please use the list below as a checklist for the submission of your documents.

Please send one original signed copy of your entire application in the following order:

The mission statement of the applicant

Partnership Agreement Narrative

Supporting documents

If the organization or agency has not applied for any other Kansas Arts Commission grant program for Fiscal Year 2012, please submit:

Current board of directors’ list with phone, address, email contact information for each member

Most recent IRS Form 990 (nonprofit organizations only) If the applicant is attached to a larger entity (i.e., a college, city or county government), no financial statement is necessary

And as required (see All Applicants):

Federal Tax-Exempt Status Letter (for first-time applicants or those with a name change)

Kansas Nonprofit Incorporation Letter (for first-time applicants or those with a name change)

Accessibility audit/checklist (first-time applicants or change in venue)
